| | | | | | | | Background:
The Houston Health Department requests that Council approve a motion to extend the unpaid leave of absence for Rocquel Hines, Environmental Investigator III, beginning June 04, 2016 through June 12, 2016.
Ms. Hines sustained a job injury on May 14, 2015. Additionally, she exhausted her FMLA for the 2015-2016 benefit year on December 6, 2015. Ms. Hines was released to full duty without restrictions by her treating physician on June 13, 2016.
Ms. Hines was approved by the Municipal Civil Service Commission for unpaid leave of absence through June 03, 2016. In accordance with Code of Ordinances 14-172, any further unpaid leaves must be approved by the City Council. Therefore, the department requests that City Council approve an extended unpaid leave of absence beginning June 04, 2016 through June 12, 2016. |
