Summary:

ORDINANCE approving and authorizing third amendments to various vendor agreements between the City and ALPHA EYECARE ASSOCIATES PLLC, ALZHEIMER’S ASSOCIATION - HOUSTON & SOUTHEAST TEXAS CHAPTER, CHINESE COMMUNITY CENTER, INC., COUNCIL FOR HEALTH EDUCATION AND DEVELOPMENT, LLC, DINSMORE MEDICAL SYSTEMS, LLC, GATEWAY TO CARE, LONE STAR LEGAL AID, MONTROSE COUNSELING CENTER, INC., HEIGHTS HEARING AIDS, LLC, RONALD L. NORSWORTHY, INC., dba HOME HELPERS OF FORT BEND, TEXAS DENTAL PROFESSIONALS CORPORATION, TEXAS SOUTHERN UNIVERSITY, and THE UNIVERSITY OF TEXAS HEALTH SCIENCE CENTER AT HOUSTON, respectively, to extend the term of the contracts for support services to the elderly provided through the Harris County Area Agency on Aging; approving and authorizing fourth amendments to various vendor agreements between the City and MPJ AUDIOLOGY CONSULTANT, LLC, and PERRYLEE HOME HEALTH CARE SERVICES, INC., respectively, to extend the term of the contracts for support services to the elderly provided through the Harris County Area Agency on Aging

Background:

 

The Houston Health Department (HHD) requests City Council to approve an ordinance authorizing amendments to extend contracts between the City and various providers of elderly services through the Harris County Area Agency on Aging from October 1, 2015 to September 30, 2016.

 

These support services providers were selected through a request for proposal process in 2007 and 2008 to provide dental screening and appliances, hearing screening and appliances, vision screening and appliances and legal assistance, to seniors (60+) who reside in the City of Houston and Harris County. The respective contracts were approved on May 9, 2007, March 11, 2009, June 10, 2009, June 17, 2009, July 29, 2009, August 26, 2009, June 16, 2010, July, 28, 2010, September 15, 2010, February 23, 2011, June 8, 2011, and August 3, 2011. Each contract was effective on the date of countersignature and extended through September 30, 2015.   

 

The City of Houston, as designated by DADS, has full authority and responsibility for funds and services authorized under the Older Americans Act of 1965, as amended. The funds are disbursed by the U.S. Department of Health and Human Services through the Texas Department of Aging and Disability Services and administered by HHD's Harris County Area Agency on Aging.
