Summary:

ORDINANCE approving the submission of a grant application to the TEXAS DEPARTMENT OF AGING AND DISABILITY SERVICES for the operation of the care connection Aging and Disability Resource Center for 2015-2016; declaring the City’s eligibility for such grant; authorizing the Director of the Houston Health and Human Services Department (“Director”) to act as the City’s representative in the application process; authorizing the director to accept the grant and expend the grant funds, as awarded, and to apply for and accept all subsequent awards, if any, pertaining to the grant

Background:
The Houston Department of Health and Human Services (HDHHS) requests City Council approval of a grant application to the Texas Department of Aging and Disability Services (DADS) to be administered by Care Connection Aging and Disability Resource Center (CC-ADRC).  The total projected award for the period from 9/1/15 – 8/31/16 is $693,704.00. The grant does not require a match.   HDHHS also requests City Council to authorized the Mayor to execute all related contracts, agreements and documents with the approval of the City Attorney in connection with the grant and to authorize the Director or his designee to act as the City’s representative in the application process with the authority to apply for, accept and expend the grant funds, as awarded, and to apply for and accept and expend all subsequent awards not to exceed five years, if any.   The funding provided by DADS will support provision of CC-ADRC services that address the needs of older adults and people with disabilities in Harris and the twelve county region of Austin, Brazoria, Chambers, Colorado, Fort Bend, Galveston, Liberty, Matagorda, Montgomery, Walker, Waller, and Wharton.  These core services include an intake line, outreach and education, community partnerships, cross-training, programs such as Money Follows the Person, Housing Navigation and Evidence-Based programming.
