The Houston Health Department (HHD) requests City Council approval of an ordinance authorizing and approving this contract, agreement, amendment, or other undertaking described in the title (Amendment 1 to Services Agreement). Amendment 1 to the service agreement of The University of Texas M.D. Anderson Cancer Center (MD Anderson) for its Be Well Acres Homes program is to implement a multicomponent obesity prevention approach to promote and support healthy eating and active living by expanding the DAWN program to Acres Home. The program will include health education focused on nutrition, physical activity and other health conditions including diabetes, hypertension, and cholesterol. The total amount of the contract is not to exceed $996,119.33 The contract duration is extended to August 31, 2024.
Specific Explanation:
The contract was amended and countersigned on September 7, 2022. The original contract was under the high value threshold of $400K thereby did not require Council approval. The FY23 and FY24 Budget amounts makes this a high value award and requires Council approval. Therefore, HHD is requesting an ordinance to approve and ratify Amendment No. 1 to extend the contract duration to August 31, 2024, and increase the contract amount.
HHD also requests City Council authorize the Mayor to execute all related contracts, agreements, amendments, and documents with the approval as to form of the City Attorney in connection with the grant application and to authorize the Director or his designee to act as the City’s representative with the authority to apply for, accept and expend the grant funds as awarded, and to accept and expend all subsequent supplemental awards, if any, and to extend the term and/or budget and project period not to exceed 5 years.
